Summary
But Did You Document It? A Recordkeeping Guidebook for Behavioral Health Professionals offers an overview of documentation practices in behavioral health and educational settings. The book addresses recordkeeping essentials for those engaged in disciplines such as psychology and behavior analysis, education, social work, counseling, and related fields. Emphasizing current requirements, the text is designed for individuals who are new to the field, as well as those seeking to refine their practice or navigate multiple professional roles across contexts.

The book is organized into seven sections covering fundamentals of recordkeeping, legal and ethical considerations, strategies for effective documentation, electronic health records, storage and disclosure procedures, quality control and audit processes, and special topics in documentation. Each chapter includes case studies that connect concepts to practice, as well as exercises to help readers assess and enhance their own recordkeeping approaches. Practical scenarios and exercises encourage adaptation to different professions and settings with consideration of documentation requirements across various licensure and practice standards.

But Did You Document It? is ideal for courses in psychology, school and clinical counseling, social work, behavior analysis, and health administration at the undergraduate and graduate levels.
